A project designed to reduce the number of unplanned hospital admissions for frequent users of health and social care services in North Staffordshire is enabling patients to receive the right care, in the right place at the right time.

Staffordshire and Stoke on Trent Partnership NHS Trust is working in collaboration with University Hospitals of North Midlands NHS Trust and local doctors to reduce unnecessary hospital admissions and the length of time patients spend in hospital.
