The CANHEART ‘big data’ research initiative is aimed at measuring and improving cardiovascular health and the quality of ambulatory cardiovascular care provided in Ontario, Canada using the population-based CANHEART cohorts. The CANHEART cohorts were created through the linkage of 19+ routinely collected health administrative, vital statistics, survey and laboratory databases housed at ICES. The strength of these cohorts lies in the large sample size, containing information on 9.8 million Ontarians age 20-105 years, and diversity of the linked databases.
Building upon the insights gained to date through the CANHEART work, we have recently launched a new CANHEART SPOR project aimed at leveraging big data to conduct innovative cardiovascular clinical trials. We aim to: 1) undertake a pragmatic cluster randomized registry-based clinical trial to improve lipid-management amongst intermediate-and high-risk patients residing in high-risk health regions in Ontario (called CHOICES), and 2) develop novel algorithms for measuring clinical outcomes in clinical trials using health-related databases and compare whether they are as accurate as traditional event ascertainment methods in a prospective clinical trial (called INVESTED). These projects will be undertaken by an interdisciplinary team consisting of experts in administrative health databases, implementation science, clinical trials, knowledge translation and patient engagement.

Una revisión sistemática exhaustiva de todas las evidencias disponibles de ensayos controlados aleatorizados sobre vacunas contra la covid-19 hasta noviembre de 2021 ha concluido que la mayoría protegen contra la infección y la enfermedad grave o crítica causada por el virus. La revisión, una colaboración de expertos independientes de varios países, también observó que hubo poca o ninguna diferencia entre el número de personas que experimentaban efectos secundarios graves después de la vacunación en comparación con las que no estaban vacunadas.

Tramadol es un opioide débil con un consumo elevado desde el momento de su puesta en el mercado en España
en el año 1998. La aparición en el mercado de combinaciones de tramadol con paracetamol y dexketoprofeno,
supuestamente a dosis bajas, ha hecho que se tenga una falsa sensación de seguridad y no se tenga conciencia de los
riesgos del consumo crónico de tramadol. En los últimos años han aparecido numerosas publicaciones haciéndose
eco de problemas de seguridad, mal uso y comercio ilícito que de este fármaco se está haciendo a nivel mundial.
El tramadol se ha incluido en el Plan de optimización de la utilización de analgésicos opioides en dolor crónico
no oncológico en el sistema nacional de salud aprobado el 13 de julio de 2021 debido a los problemas de seguridad
identificados que conlleva el uso crónico de este fármaco. La prevalencia de consumo de tramadol a nivel nacional
en el grupo de 15-64 años en el año 2019 fue de 7,2% (6,3 en hombres y 8,2 en mujeres1
).

The relationship between large-group classroom attendance by students and test achievement in problem-based learning (PBL) curricula is unclear. This study examined the correlation between attendance at resource sessions (hybrid lectures in the PBL curriculum) and test scores achieved in pharmacology and determined whether the score achieved was related to student gender. A cross-sectional observational study over one academic year of 1404 pre-clerkship medical students was performed. Class attendance during pharmacology resource sessions and MCQ test scores achieved in pharmacology were analysed. The percentage of students’ attendance in resource sessions declined over three years of the programme, from 78.7 ± 27.5 in unit I to 22.1 ± 35.6 (mean ± SD) in unit IX. A significant but weakly positive correlation was evident between attendance and achievement in pharmacology (r = 0.280; p < 0.0001). The mean score of the students who attended > 50% of the resource sessions was significantly higher (p < 0.0001). Students who attended ≤50% were more likely to achieve lower tertile scores. The mean score achieved and the number of higher tertile scorers were higher among students who attended > 50% of the resource sessions. Although female students’ attendance was significantly higher, no significant gender-related differences in either mean scores or top grades achieved were found. In a PBL curriculum, the classroom attendance of students in pharmacology declined during the pre-clerkship phase. A weak positive correlation was found between attendance and academic achievement, as measured by MCQ test scores. Factors other than motivation and attendance may confound gender-based academic performance and merit further research.
