This is an important study that contributes to the evidence-base for a higher incidence rate of dementia among women compared to men, and a pooled higher risk of dementia among women. Other research has attributed increased levels of dementia among post-menopausal women to a potential detrimental neurological effect of lack of oestrogens (Rahman et al, 2020), while androgens are purported to have neuroprotective effects (Cai et al, 2020). However, the inconsistency of effect estimates seen by Gong et al. (2022) across cohorts suggests that, if a difference in dementia risk between men and women truly exists, this may not be fully explained by hormonal or biological factors.
Substance use and the mental health consequences in trans and non-binary individuals is currently still understudied, although these individuals are at increased risk of mental illnesses. This study highlights that alcohol use is increased in this community, and is associated with risk factors, using patterns and motives of alcohol use differ based on gender.
The cornerstone of this study is it’s contribution to our learning about what works for whom – and when it works best. Having personally discussed with many clients and ex-clients the changes to service provision that COVID-19 brought, the recurring main theme is the importance of patient choice and the concern that services who are still working predominantly or exclusively remotely are not offering this. We have gained a lot from digital developments during this period, which has increased access to therapy for many people; but we must not let this overshadow the preferences of others who may need other adaptations to ensure they can access services in a timely and appropriate way.
As said by participants in this survey, there are key systemic issues that need addressing as a priority. Food insecurity (a lack of economic, social or physical means to ensure a sufficient supply of nutritionally appropriate food; Purdam et al., 2016) is at an all-time high in the UK and greatly affects those with EDs (Hazzard et al., 2023; Kuehne et al., 2023). Introducing measures that contribute to a culture of shame and anxiety around food will exacerbate the guilt and shame already experienced by those affected by food insecurity and eating disorders. Adding calorie information on menus feels like an extra cruel addition to the hostile environment those recovering from an ED already find themselves in.
this study contributes to the growing evidence-base which suggests that ACT could be another treatment option for people with psychosis. Further studies of ACT for people with psychosis are required to continue building upon the evidence-base.
Within healthcare services, we facilitate ACT-based groups for people with mental health difficulties. We have an opportunity here to set up ACT-based groups specifically for people with psychosis and conduct service evaluation studies to evaluate their feasibility in services.
