Stoke on Trent and North Staffordshire CCGs have rolled out a free self care app for long term condition patients. The app, which uses avatars developed by Keele University to demonstrate self care advice is being used to support a range of long term health conditions including asthma, COPD, hypertension, Type 2 Diabetes, lower back pain, stroke/TIA and Atrial Fibrillation.
