Efficacy of schizophrenia treatments has changed little since the discovery of chlorpromazine more than 50 years ago. Generally, antipsychotics produce a moderate improvement in symptoms (SD 0·3–0·6) versus placebo.1 The atypical antipsychotic clozapine produces an additional benefit of around SD 0·5 over traditional agents, but attempts to develop additional compounds that replicate the effects of clozapine have consistently failed.
