Whether euthanasia or assisted suicide should be legalised is one of the most pressing and profound questions facing legislators, health-care professionals, their patients and indeed all members of society. Regrettably, the debate is too often characterised by rhetoric rather than reason. This book aims to inform the debate by acquainting anyone interested in this vital question with some of the major ethical, legal and clinical and theological issues involved. The essays it contains are authoritative, balanced and readable: authoritative in that they have been commissioned from some of the world's leading experts; balanced in that they reflect divergent viewpoints (including a vigorous debate between two eminent philosophers); and readable in that they should be readily intelligible to the general reader. The chairman of a Dutch suicide support group has been given a prison sentence and his organisation fined in a test ruling that highlights that doctors alone can assist suicide in the Netherlands. Gerard Schellekens, of the Foundation for a Voluntary Life (SVL), helped an 80 year old woman, who was bedridden with advanced Parkinson’s disease commit suicide after the GP at her nursing home refused euthanasia.
Introduction: In The Netherlands, physicians have to be convinced that the patient suffers unbearably and hopelessly before granting a request for euthanasia. The extent to which general practitioners (GPs), consulted physicians and members of the euthanasia review committees judge this criterion similarly was evaluated.

But there is evidence that some clinicians may already be using continuous deep sedation (CDS), as a form of "slow euthanasia". Research suggests use of CDS in Britain is particularly high - accounting for about one in six of all deaths.
Proponents of assisted suicide believe support for legalisation is growing among lawmakers and the public around the world. In the past year three names have been added to the list of places which permit it. The BBC's Vincent Dowd investigates whether assisted suicide is set to become even more common.
Objectives: To investigate why physicians label end-of-life acts as either ‘euthanasia/ending of life’ or ‘alleviation of symptoms/palliative or terminal sedation’, and to study the association of such labelling with intended reporting of these acts. Conclusions: Similar cases are not uniformly labelled. However, a physicians’ label is strongly associated with their willingness to report their acts. Differences in how physicians label similar acts impede complete societal control. Further education and debate could enhance the level of agreement about what is physician-assisted dying, and thus should be reported, and what not.
In the legal performance of the euthanasia procedure, unbearable suffering, one of the requirements of due care, is difficult to assess. Evaluation of the current knowledge of unbearable suffering is needed in the ongoing debate about the conditions on which EAS can be approved. Using an integrative literature review, we evaluated publications with definitions of suffering in general or in end-of-life situations and with descriptions of suffering in the context of a request for EAS.
Battin et al examined data on deaths from PAS in Oregon and on PAS and VE in The Netherlands. This paper reviews the methodology used and questions the conclusions drawn from it—namely, that there is for the most part ‘no evidence of heightened risk’ to vulnerable people from the legalisation of PAS or VE. This critique focuses on the evidence about PAS in Oregon. It suggests that vulnerability to PAS cannot be categorised simply by reference to race, gender or other socioeconomic status and that the impetus to seek PAS derives from factors, including emotional state, reactions to loss, personality type and situation and possibly to PAS contagion, all factors that apply across the social spectrum. It also argues that the highest resort to PAS in Oregon is among the elderly and that some terminally ill patients in Oregon are taking their own lives with lethal drugs supplied by doctors despite having had depression at the time when they were assessed and cleared for PAS.
In their critique of our paper "Legal physician-assisted dying in Oregon and the Netherlands: evidence concerning the impact on patients in "vulnerable" groups," I.G. Finlay and R. George claim to challenge our underlying assumptions and methodology with "another perspective on Oregon's data." In our view, however, they miss the point of our paper and address a quite different issue. While we welcome their attempt to further explore issues about assisted dying, we do not believe they have in any way undercut our argument that where assisted dying is already legal (at the time of our study, Oregon and the Netherlands), there is no current evidence for the claim that legalized physician-assisted suicide or euthanasia will have disproportionate impact on patients in vulnerable groups.
