After anesthesia, medication safety has perhaps been the most closely studied domain in patient safety. Efforts to reduce the rate of medication errors have involved all the strategies discussed above. Nearly half of serious medication errors have been found to result from the fact that clinicians have insufficient information about the patient and the drug.
