There is insufficient randomized controlled trial evidence to draw conclusions regarding the benefits or harms of antipsychotic medications for cocaine dependence.
Moderate-quality evidence showed that fewer people with cocaine dependence receiving an antipsychotic drug withdrew from treatment when compared with placebo (on average, 411 versus 547 withdrew per 1000 people), implying some benefit of antipsychotic drugs. However, this is a surrogate outcome and withdrawal from a trial can be for reasons other than lack of effectiveness or adverse effects.
Relatively little is known about the temporal relation between at-risk gambling or problem gambling (PG) and mental and substance use disorders (SUDs) in young adulthood. The regulation of new psychoactive substances (NPS) has confounded governments throughout the western world. In 2014 the UK government convened an NPS Review Expert Panel to consider a range of approaches. Ultimately the Panel recommended that the government ban all new psychoactive drugs and allow only psychoactive substances specifically exempted, such as alcohol, tobacco and those allowed as medicines. The government introduced the Psychoactive Substances Bill (PSB) in response to that recommendation. Passed in 2016, the Bill has attracted a torrent of criticism from scientists and experts. The Bill could be improved with revision, but the problems of the total ban, as envisioned by the PSB, with respect to the NPS, may be inherent: (1) defining psychoactivity is conceptually fraught, with great consequence for the scope of the prohibition; (2) operationalizing psychoactivity as a usable concept for legal control purposes is extremely difficult, perhaps impossible; and (3) the detachment of penalties for violating a total ban from establishing the harmfulness of a substance is normatively troubling. Given the uncertainties about the effects of a total ban, it is appropriate at this time for other governments to assess more fully the nature of the NPS problem, and the potential control approaches. In a recent study published in Addiction, the authors set out to examine the efficacy and tolerability of adjunctive pharmacotherapies for smoking cessation in adults suffering from serious mental illness. Pharmacotherapies included bupropion, varenicline and NRT delivered alone or in combination.
On June 7, the US Food and Drug Administration released a drug safety notice about loperamide, an anti-diarrhoea medicine sold over the counter and under prescription. The drug is being used as a cheap and legally-available high, and to reduce the symptoms of opioid withdrawal. A standard dose for the medicine is 8 mg per day in divided doses, but opioid users have been taking massive doses of the drug, as much as 300 mg or more daily for weeks at a time. Taken in such high quantities, overdoses of loperamide can cause severe heart problems like ventricular arrhythmias, cardiac arrest, and death. Although the number of reported cases is currently small, standard toxicology screenings do not look for loperamide, and it is possible that the rate of loperamide abuse is under-reported. Smoking is one of the major modifiable risk factors contributing to early mortality for people with serious mental illness. However, only a minority of service users access smoking cessation interventions and there are concerns about the appropriateness of generic stop-smoking services for this group. The SCIMITAR (Smoking Cessation Intervention for Severe Mental Ill-Health Trial) feasibility study explored the effectiveness of a bespoke smoking cessation intervention delivered by mental health workers. Research using different behavioral economic (BE) and time perspective (TP) measures suggests that substance misusers show greater sensitivity to shorter term contingencies than normal controls, but multiple measures have seldom been investigated together. This study evaluated the extent to which multiple BE and TP measures were associated with drinking problem severity, distinguished initial outcomes of natural recovery attempts, and shared common variance. Hypotheses were (1) greater problem severity would be associated with greater impulsivity and demand for alcohol and shorter TPs; and (2) low-risk drinking would be associated with greater sensitivity to longer term contingencies compared with abstinence. Attention-deficit/hyperactivity disorder (ADHD) is associated with health risks in adolescence which includes the potential for smoking cigarettes, early smoking initiation, and rapid progression to daily smoking. Much less is known, however, about prognostically relevant smoking behaviors among individuals with childhood ADHD. Further research in this area is important for identifying individuals at pronounced risk for nicotine addiction, and for developing effective interventions for this population. Patients with chronic pain, depression, and substance use disorder (SUD) are often treated in primary care settings. An estimated 52% of patients have a diagnosis of chronic pain, 5% to 13% have depression, and 19% have SUD. These estimates are likely low when considering the fact that 50% of primary care patients with depression and 65% with SUD are undiagnosed or do not seek help. These three conditions have overlapping neurophysiological processes, which complicate the treatment outcomes of a primary physical illness. Behavioral interventions have been widely utilized as adjunctive treatments, yet little is known about what types of behavioral interventions were effective to treat these comorbidities. Co-occurring substance use increases the risk of hospitalisation in people with severe mental illness, whereas Assertive Community Treatment (ACT) generally reduces hospitalisation in patients with severe mental illness and high inpatient service use. Because the superiority of ACT over standard services amongst patients with problematic substance use is uncertain, the present study examined inpatient service use amongst patients with and without problematic substance use in the 2 years before and the 2 years after they enrolled into ACT teams in Norway.
The impact of cannabis (Hamilton, 2016) on mental health (Kennedy, 2015) is of particular interest in the USA, where cannabis is now legal in some states, and decriminalized in a number of others. There is a fear that cannabis use will increase, and therefore there is a pressing need to understand the nature of its association with psychiatric problems.
Blanco and colleagues state that this is their particular motivation for undertaking the research they have just published, to try and understand whether cannabis use predicts later substance use disorders, and also mood and anxiety disorders.
Psychotic experiences (PE) are commonly reported in the general population, where they are associated with elevated clinical need and functional impairment. Substance abuse is a growing, but mostly silent, epidemic among older adults. We sought to analyze the trends in admissions for substance abuse treatment among older adults in the US (aged 55 and older).
Internet pornography use is an increasingly common, yet controversial, behavior. Whereas mental health communities are divided about potentially problematic use patterns, many laypeople identify as feeling dysregulated or compulsive in their use. Prior work has labeled this tendency perceived addiction to internet pornography (PA). Estimating ‘return on investment’ (ROI) from smoking cessation interventions requires reach and effectiveness parameters for interventions for use in economic models such as the EQUIPT ROI tool (http://roi.equipt.eu). The objective of the study was to describe self-report and objectively measured sleep characteristics of adult treatment-seeking cannabis users. Study participants (n = 87) were adults who were screened for a 12-week outpatient cannabis treatment research program in Baltimore, MD. Providing stop smoking services to people with substance misuse disorders increases the numbers of people who stop smoking by about 10% without reducing the rates of abstinence from drugs or alcohol. Combined drug treatment and counselling showed the best result though pharmacotherapy alone was also successful. However counselling alone was not beneficial.
