It is generally believed that after the first episode of schizophrenia, the risk of relapse decreases with time in patients who are stabilized. Many treatment guidelines recommend that after stabilization, antipsychotic treatment should be continued for 1–5 years, and longer exposure should be avoided if possible. However, there is no published evidence to substantiate this view. People with schizophrenia are at risk of receiving poorer end of life care than other patients. They are often undertreated, avoid treatment and are about half as likely to access palliative care. There are limited options for end of life care for this under-serviced group in need. This study aims to address the paucity of research by documenting possible need, experiences of health care service use and factors affecting palliative care use for people with schizophrenia who have advanced life limiting illness.

The game uses a binaural technique that mimics 3D human hearing – players experience visual and auditory hallucinations as if they are Senua and 'hear' voices just behind them, or whispering in their ear.

There is rising awareness that we need multidisciplinary approaches integrating psychological treatments for schizophrenia, but a comprehensive evidence based on their relative efficacy is lacking. We will conduct a network meta-analysis (NMA), integrating direct and indirect comparisons from randomised controlled trials (RCTs) to rank psychological treatments for schizophrenia according to their efficacy, acceptability and tolerability.

Ritualistic behaviors are common in obsessive compulsive disorder (OCD), while catatonic stupor occasionally occurs in psychotic or mood disorders. Schizoaffective disorder is a specific mental disorder involving both psychotic and affective symptoms. The syndrome usually represents a specific diagnosis, as in the case of the 10th edition of the International Classification of Diseases (ICD-10) or the 5th edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). However, symptom-based diagnosis can result in misdiagnosis and hinder effective treatment. Few cases of ritualistic behaviors and catatonic stupor associated with schizoaffective disorder have been reported. Risperidone and modified electroconvulsive therapy (MECT) were effective in our case.

Recent evidence has suggested that psychosis could develop not only in people at clinical high risk for psychosis (CHR-P) but also in those with clinical risk syndromes for emergent nonpsychotic mental disorders. Marie Stenlund’s careful reading of values-based practice and her demonstration of its links with Martha Nussbaum’s Capabilities Framework are innovative theoretically and have potentially important implications for policy and practice in mental health. Associations between serious mental disorder and violence are well-documented, but there is little epidemiological evidence linking these disorders and homicide risk. The reported study compares socio-demographic and clinical characteristics of people diagnosed with schizophrenia who committed homicide vs. those who died by suicide. The study is a national case series of male patients in England & Wales diagnosed with schizophrenia and convicted of homicide during 1997–2012 (n = 168), and a randomly selected comparison group of male patients with schizophrenia who died by suicide and who were matched to the homicide case series by age (n = 777).. Experiential negative symptoms—including diminished motivation—have a profound impact on functional outcomes in schizophrenia. Animal research suggests that abnormalities in dopaminergic regulation can negatively impact effort exertion, a translational model that has been applied to individuals with schizophrenia. Paradigms that assess effort-based decision making, for example, suggest less likelihood of choosing high effort tasks that are high in probability of success, and this preference varies with negative symptoms and impaired functioning. Although asociality is another well-documented component of experiential negative symptoms, it is unclear whether diminished motivation for monetary reward extends to the social domain.. Outpatient facilities, such as community behavioral health organizations (CBHOs), play a critical role in the care of patients with serious mental illness, but there is a paucity of “real-world” patient outcomes data from this health care setting. Therefore, we conducted The Research and Evaluation of Antipsychotic Treatment in Community Behavioral Health Organizations, Outcomes (REACH-OUT) trial, a real-world, prospective, noninterventional observational study of patients with mental illness treated at CBHOs across the United States. Building on support for the five-factor model (FFM) of personality disorder, the Five Factor Schizotypal Inventory (FFSI) was developed to assess maladaptive traits relevant to schizotypal personality disorder. While the development of the FFSI supports a continuity between schizotypal thinking and perception (STAP) and the FFM domain of Openness to Experience, other studies show inconsistent findings concerning the strength of this relationship. I have endured auditory and visual hallucinations, paranoid thoughts, and depressive symptoms since the age of 6 (some 40 years ago), and was formally diagnosed with schizoaffective disorder, depressive type in young adulthood. I chose not to surrender to my illness, but instead to wage a war against my symptoms. And I felt that I was winning that war until I chose to leave a successful but stressful career (built over a period of 25 years) to restore my health and my quality of life. . Patients with schizophrenia are often plagued by sleep disturbances that can exacerbate the illness, including potentiating psychosis and cognitive impairments. Cognitive dysfunction is a core feature of schizophrenia with learning and memory being particularly impaired. Sleep disruptions often accompanying the illness and may be key mechanism that contribute to these core dysfunctions. In the era of deinstitutionalization, employment was an uncommon outcome for people with schizophrenia and other severe mental illnesses living in the community.1 Mental health and vocational rehabilitation professionals initially offered little help, perhaps because they believed that people with severe mental illnesses were unable or unlikely to work competitively. Instead, professionals referred people coming out of hospitals to sheltered settings, such as day programs and sheltered workshops, where they could be monitored closely and trained to work in a slow, step-wise fashion. Improving access to psychotherapies in psychosis requires workforce expansion in resource-challenged systems. The GOALS feasibility randomized controlled trial assessed training and implementation of an evidence-based intervention by frontline workers, targeting recovery goals. Training uptake and therapy fidelity were good. Case managers with crisis management responsibilities were less likely than clinical assistants to deliver therapy. Participants receiving “sufficient therapy” achieved goals, but therapy was usually provided by clinical assistants. This is consistent with implementation science principles, that training must be combined with supportive organizational structures, such as by focusing on roles that already include therapy delivery or developing stronger organizational supports for case managers.. This paper describes a reflective space created between two Singaporean Chinese systemic psychotherapists and a British Chinese family – the Chans. The family attended the Marlborough Cultural Therapy Centre (MCTC) in London and was given two separate culture-specific systemic treatments by different therapists, which were reflected upon collaboratively with the Chans. A brief literature review of British Chinese and mental health, culture and reflexivity is described. The authors discuss clinical implications including the impact of British Chinese cultures and the effect of mental health issues on the family. The process and outcomes of the treatment are explained using the frame of reflexivity. A recent years' increase in both prescribing and availability of second-generation antipsychotics (SGAs) has been observed. According to the literature, typically made up by case studies/series, quetiapine seems to be the most commonly misused SGA, with both intranasal and intravenous intake modalities having been described. Another SGA that has been anecdotally reported to be misused is olanzapine. For these molecules, both a previous history of drug misuse and being an inmate have been described as factors associated with misuse. Hence, while providing here an updated literature review of the topic, we aimed at assessing all cases of quetiapine misuse/abuse/dependence/withdrawal as reported to the European Medicines Agency's EudraVigilance (EV) database; this was carried out in comparison with the reference drug olanzapine.. Schizophrenia and bipolar disorder (BD) are heritable disorders with similarities in clinical symptoms and typical onset after puberty.1 While research shows that impaired motor coordination can have an association with schizophrenia,2 there are limited data on childhood development preceding BD. Comparison of antipsychotic combinations (different combinations of clozapine, atypical and typical antipsychotics) versus the same antipsychotics in monotherapy revealed no apparent differences in relapse, hospital admission, global state, mental state, withdrawal for any reason, adverse events (serious or requiring discontinuation, movement disorder, weight gain, tremor), or quality of life. When assessed, the evidence is of very low to low quality. Antipsychotic combinations may result in better clinical response than antipsychotic monotherapy, but the quality of evidence is very low.
