Lifelong effects include growth restriction, characteristic facial anomalies, and neurobehavioral dysfunction. This range of effects is known as fetal alcohol spectrum disorders (FASD). There is no amount, pattern, or timing of alcohol use during pregnancy proven safe for a developing embryo or fetus. Therefore, it is important to screen patients for alcohol use, inform them about alcohol's potential effects during pregnancy, encourage abstinence, and refer for intervention if necessary. However, how and how often nurses and midwives inquire about alcohol drinking during pregnancy or use recommended screening tools and barriers they perceive to alcohol screening has not been well established. Over the past decade, psychedelic compounds have emerged as potentially transformative therapeutics for a variety of intractable neuropsychiatric conditions. However, historically most of the basic science has utilized these compounds as probes to interrogate various endogenous neurotransmitter systems—mainly the serotonin 5-HT2A receptor. With the renewed interest in utilizing these compounds as therapeutics and the explosion in clinical trials, psychedelics have been purported to treat many neuropsychiatric disorders, including depression, cluster headaches, migraines, anxiety, and obsessive-compulsive disorder. It is therefore imperative to understand the biology and pharmacology behind their therapeutic mechanisms as well as expose any potential pitfalls in their widespread use as treatments. The first offenders have been jailed under the new powers, which were introduced to tackle the menace of so-called legal highs – with more cases currently progressing through the criminal justice system.

It is widely recognized that substance abuse increases the general risk of delinquency and violent offenses in particular. Substance-related disorders are the primary cause of offenders being sentenced to mandatory treatment in forensic psychiatry. Psychosis can be associated with acute recreational drug and novel psychoactive substance (NPS) toxicity. However, there is limited data available on how common this is and which drugs are most frequently implicated. We describe a European case series of psychosis associated with acute recreational drug toxicity, and estimate the frequency of psychosis for different recreational drugs.
The study reviews publications on the use of methamphetamine and amphetamine in relation to psychotic symptoms, substance-induced psychosis, and primary psychosis published between July 2014 and December 2015. Comment. In The Lancet Psychiatry, Schoeler and colleagues present a study1 describing the mediating effect of medication adherence on the association between continued cannabis use and relapse risk in patients with first-episode psychosis. They have previously reported a relapse rate of 36% in this patient group over a 2-year period.2 Acknowledging the potential risk of psychosis relapse related to the high proportion of patients continuing cannabis use after the onset of psychosis, the current study1 investigates the same patient group consisting of 245 patients, obtaining retrospective data on active cannabis use and medication adherence shortly after illness onset, as well as risk of relapse at 2-year follow-up. The authors find that relapse of psychosis associated with continued cannabis use is partly mediated through non-adherence to prescribed antipsychotic medication. For a decade, cannabis use in the population has been in decline, although it is still the most popular illicit drug. A recent report suggests this trend of decreasing use is turning as cannabis use has increased across Europe (EMCDDA 2016).
Irrespective of these broader population trends there has been a dramatic increase in demand for treatment as a result of people developing problems related to cannabis use (Hamilton et al 2014). New presentations to drug treatment services (where cannabis is the primary drug used) have grown by 49% in the 8 years from 2005-2013.
Given the rapid increase in demand for help there is an urgent need to establish a choice of evidence-based treatments. This will help guide policy makers, treatment providers and most importantly individuals who experience problems with cannabis.
So it is timely that a Cochrane systematic review has been published that seeks to pull together what is known about psychosocial interventions for cannabis use disorder (Gates et al, 2016).
Clinical guidelines from around the world recommend the delivery of psychosocial interventions as part of routine care in opiate substitution treatment (OST) programmes. However, although individual studies demonstrate benefit for structured psychosocial interventions, meta-analytical reviews find no benefit for manual-based treatments beyond ‘routine counselling’. More people with substance misuse issues in Cannock will soon get access to better support to help them turn their lives around, after a treatment centre was given the green light by planning officials.
Addiction Dependency Solutions, in partnership with Staffordshire County Council, will be developing the centre to accommodate those who are undergoing treatment for alcohol, substance and steroid misuse.
The centre will also provide support on mental health issues, housing, prescribing clinics, and support for family and carers, and is the first step towards accessing the ongoing recovery services offered by the BAC O’Connor Centre in the town.
The services are all part of the Staffordshire County Council commissioned ‘One Recovery’ service. One Recovery is led by Addiction Dependency Solutions (ADS), and supported by North Staffordshire NHS Combined Healthcare Trust, Changes, Brighter Futures and Arch Housing.
In Cannock Chase alone, 474 people are currently accessing drug and alcohol treatment services, the highest number in Staffordshire. Statistics show that Cannock is in the worst 25% of areas in the country for deaths directly due to alcohol. Added to this, the area is in the top 5% for alcohol related hospital admissions for people under 18.
Cannabis is a public health issue that is not going away. In the USA, cannabis use has doubled in the past ten years,1 with approximately 22 million American users in 2014.2 The risks of both short-term and long-term cannabis use have been well described. Short-term use can result in impaired memory, motor coordination, and decision making.3 Long-term use can affect brain development, functional outcomes, and mental health.3 In the face of these risks, as states and countries consider enactment of laws relating to medical and recreational cannabis use, clear and evidence-based cannabis policies are needed. A previously healthy 21-year-old woman presented with subacute onset of confusion and gait ataxia. She reported habitual inhalation of nitrous oxide (N2O) purchased online over the past year, with increased consumption over the preceding weeks of up to 300 canisters/week. Examination revealed pale skin with diffuse hyperpigmented macular patches over the trunk (figure 1) and atrophic glossitis. Cognitive impairment was evident with impaired insight, orientation, short-term memory and attention. She had impaired limb proprioception with sensory ataxia and positive Romberg’s test. Despite the relative success of CBT for treating substance use disorders, effective human laboratory models of CBT are lacking. Extant reviews on this association focused on a restricted range of substance-related addictions, and/or used a narrative instead of a systematic approach.
Previous research has found that area-level income inequality and individual-level relative deprivation both contribute to disordered gambling in adults. However, the socioeconomic factors that contribute to disordered gambling in youths and protective factors in their social environment have not been fully explored. This study examined the association between relative deprivation and youth disordered gambling and the potential moderating role of social support in this association.

Editorial. This issue of JAMA Psychiatry includes a timely article by Grant et al1 that makes a compelling case that the United States is facing a crisis with alcohol use, one that is currently costly and about to get worse. The article reminds us that the chilling increases in opioid-related deaths2 reflect a broader issue regarding additional substance-related problems. We would like to thank Drs Naudet, Granger and Braillon for their interest in our work, and for opening a discussion that is of interest here: the consideration of complementary evidence for health technology assessment (HTA) decision-making. By ‘complementary evidence’, we refer to clinical trials, patient’s preference and quality of life, clinical guidance, current clinical practice, and public health, cost-effectiveness and budget impact assessments. As such, it should be understood that a cost-effectiveness analysis, such as the one discussed here, represents an element of evidence that is weighted in relation to a continuum of complementary evidence when assessed by HTA decision makers.
