NHSCC’s publication ‘Excellence in commissioning diabetes care’ showcases examples of where clinically led commissioning is changing the way diabetes care is commissioned and improving people’s lives.
The Portsmouth Super Six model of care is one of those which tried to answer the million dollar question: which bits of diabetes care need to be in hospitals?
In the main, it boils down to areas of care which need multidisciplinary care, access to investigations or are specific issues related to a hospital, such as antenatal care, inpatient diabetes to name just a couple.
Local diabetic eye screening providers are advised to implement the competencies in this document and any additional competencies relevant to their local service. This will help to make sure administrative work is carried out effectively and appropriately.
In an expansion of a pioneering NHS programme, tens of thousands of people will be offered education and lifestyle coaching to reduce their risk of Type 2 diabetes.
This forms part of a package of new measures to curb diabetes pledged by the Chief Executive of NHS England, Simon Stevens. This includes funding more specialist nurses, providing diabetic foot care teams and a major evaluation of how digital technology such as apps and wearable technology could provide bespoke and easy-to-access support to many millions more people at risk.
To help ensure we deliver excellent type 1 diabetes services across the capital, the London Diabetes Clinical Network has just published a comprehensive commissioning pack, containing recommendations to assist in the commissioning and delivery of excellent services, self-assessments to establish current gaps in commissioning and delivery, and performance targets expected which will ensure we deliver improved patient reported outcomes.
In 2013, NHS North East Essex CCG commissioned Suffolk GP Federation as a lead provider for a new model of primary care for adults with diabetes.
Suffolk GP Federation has a single, five year contract with the CCG to provide care — including secondary care services & a new community-based diabetes specialist team — and organise other providers across the care pathway through subcontracts.
The CCG used a number of methods to achieve the change in care, including stakeholder analysis, setting a patient-focused vision of care, and providing dedicated time for all stakeholders to give meaningful input into the design of the new model.
One year from the launch, the federation has significantly increased the number of people getting all eight necessary care practices from 40.1% to 60.3%.
The programme has also improved cholesterol and blood pressure outcomes, and decreased hospital readmissions due to diabetic ketoacidosis and hypo/hyperglycaemia by 31.6%.
