In this paper, we argue for the importance of unsettling dominant narratives in the current terrain of harm-reduction policy, practice and research. To accomplish this, we trace the historical developments regarding the Human Immunodeficiency Virus (HIV), the Hepatitis C Virus (HCV) and harm-reduction policies and practice. We argue that multiple historical junctures rather than single causes of social exclusion engender the processes of marginalisation, propelled by social movements, institutional interests, state legislation, community practices, neo-liberalism and governmentality techniques.

Two conditions that are highly stigmatized and that can be found in combination with one another are the stigma of substance use and HIV-related stigma. In this qualitative study, we investigated 15 substance users’ experiences surrounding their HIV status from their diagnosis to their disclosure decisions and the reactions from others. We explored the layering of substance use stigma with HIV-related stigma, the consequences of stigmatizing experiences, and coping strategies employed to deal with stigmatization. This study aimed to 1) identify the characteristics of individuals with substance use disorders (SUDs) who make high use of services provided by general practitioners (GP) and psychiatrists while receiving services concurrently from an addiction rehabilitation center (ARC), and 2) to compare high service users to moderate and low service users.
Smoking in the general population is on the decline. However, in people with severe mental disorders (SMDs) such as schizophrenia and bipolar disorder the estimated smoking prevalence rate is anywhere between 50-80%. High rates of medical morbidity and severely reduced life expectancy are associated with increased prevalence rates in these individuals.
Pharmacological treatments for smoking cessation are effective in the general population. We have blogged extensively about the efficacy, tolerability and cost-effectiveness of the three main treatments; transdermal nicotine patches (TNP), varenicline and bupropion.
There is emerging evidence to suggest individuals with SMDs are often motivated to quit and pharmacological treatments in those with SMDs are similarly effective. Despite these promising findings, treatments are often under-utilised, with one study finding less than one third of clinicians advising patients about smoking cessation.
Problematic alcohol use is associated with neuropsychological consequences, including cognitive biases. The goal of the study was to explore the moderating role of executive control and readiness to change on the relationship between alcohol use and cognitive biases in light and problematic drinkers with and without mild to borderline intellectual disability (MBID). Addiction is associated with severe economic and social consequences and personal tragedies, the scientific exploration of which draws upon investigations at the molecular, cellular and systems levels with a wide variety of technologies. Magnetic resonance imaging (MRI) has been key to mapping effects observed at the microscopic and mesoscopic scales. The range of measurements from this apparatus has opened new avenues linking neurobiology to behaviour. This review considers the role of MRI in addiction research, and what future technological improvements might offer.
