Intervention goals focused on optimizing performance of activities of daily living (ADLs), targeting the morning and evening self-care routines; decreasing agitation; increasing activity engagement, including physical activity, to promote sleep; and improving caregiver communication and cuing skills. Tamara reviewed the evidence from the September/ October 2017 issue of the American Journal of Occupational Therapy (Jensen & Padilla, 2017; Piersol et al., 2017; Smallfield & Syrovatla-Keckenlaible, 2017) and also read the intervention recommendations with strong evidence from the Occupational Therapy Practice Guidelines for Adults With Alzheimer's Disease and Related Major Neurocognitive Disorders (Piersol & Jensen, 2017). . Due to the demographic change, the global prevalence of dementia will continually rise. Barriers to diagnosis and care are still high. But timely diagnosis is associated with valuable benefits and can promote timely and optimal management. Receiving an early diagnosis is especially in rural areas a problem due to the limited access to assessments. Therefore, the aim of our scoping review is to investigate different interventions targeted at rural living elderly to screen and diagnose cognitive decline and dementia. . Accumulating evidence indicates that the pathophysiological process underlying Alzheimer disease—including protein aggregation, synaptic dysfunction, and neuronal loss—begins years if not decades prior to the onset of clinical symptoms and functional limitation. This long prodromal period represents our greatest hope for effective therapeutic intervention as well as a domain of serious ethical concern. Interventions to arrest or even to delay the neurodegenerative process could profoundly reduce the anticipated societal burden of Alzheimer disease in an aging global population,1 especially if initiated prior to the development of functional impairment.. Editorial commentary. This JNNP meta-analysis, which included 15 studies with 812 047 participants, showed an elevated dementia risk in lifelong single (42%) and widowed (20%) but not divorced persons compared with those who were married.. Caregivers of patients with dementia experience physical and mental deterioration. We have previously reported a correlation between caregiver burden and the Frontal Assessment Battery (FAB) total scores of patients with Alzheimer's disease (AD), especially regarding the dependency factor from the Zarit Burden Interview. The present study aimed to identify an objective biomarker for predicting caregiver burden.. To read the full article, log in using your SSSFT NHS OpenAthens details. Open access. It remains unclear to what extent postoperative delirium (POD) affects the incidence of dementia in hip fracture patients, and the methods used to detect delirium and dementia require validation. The aim of this study was to investigate the development of dementia within 3 years of femoral neck fracture repair surgery, with a focus on POD as a potential predictive factor.

This study adds to the current body of research that a healthy, balanced diet could potentially slow memory loss. But the study didn't measure dementia rates.
The study has some strengths, such as using a standardised assessment of memory at regular intervals and using a standardised questionnaire.
But it also has some limitations that mean we can't say with any certainty that leafy green vegetables could prevent memory loss, let alone dementia
Implications for practice and research:
> As caring for a partner with dementia has a negative impact on caregivers’ health, supportive interventions need to be offered to active caregivers.
> Patients with dementia are less likely to receive palliative care, so these services need to be made more accessible to patients with dementia and their caregivers.
> Comparisons between dementia bereaved partners and non-dementia bereaved groups need to be extended to other bereavement-related health problems, including complicated grief.

Open access. There is growing evidence for the role of systemic inflammation in Alzheimer’s disease (AD) and other neurodegenerative diseases; however the systemic inflammatory profile in dementia with Lewy bodies (DLB) has never before been investigated. This study aimed to characterise systemic inflammatory mediators in established DLB and AD, as well as in their prodromal, mild cognitive impairment (MCI) phases.
Open access. Editorial. The great advance of neuroimaging techniques witnessed in the last 10 years has enabled an intensive debate on the role of white matter (WM) lesions in the pathophysiology of AD. Once regarded by the early literature as a common result of cerebrovascular pathology, WM alterations, particularly those found in predementia stages, have been consistently linked to neurodegeneration processes by recent multimodal studies.........
