The department has developed a special appeals mechanism, called the special category mechanism (SCM), for those with chronic hepatitis C infection (stage 1). The SCM is a significant new element of the infected blood reforms and the government is consulting on the details of it and proposals for ensuring the scheme remains within its budget.

Cannabis use and related problems are on the rise globally alongside an increase in the potency of cannabis sold on both black and legal markets. Additionally, there has been a shift towards abandoning prohibition for a less punitive and more permissive legal stance on cannabis, such as decriminalisation and legalisation. People who inject drugs (PWID) experience high incarceration rates, and previous incarceration is associated with elevated hepatitis C virus (HCV) transmission risk. In Scotland, national survey data indicate lower HCV incidence in prison than the community (4.3 versus 7.3 per 100 person-years), but a 2.3-fold elevated transmission risk among recently released (< 6 months) PWID. We evaluated the contribution of incarceration to HCV transmission among PWID and the impact of prison-related prevention interventions, including scaling-up direct-acting antivirals (DAAs) in prison. People with concurrent disorders (CDs; i.e., mental illness and substance use disorders) are at high risk for a number of adverse outcomes if they do not receive appropriate treatment. Yet despite wide availability of evidence-based guidelines and easy-to-access clinical resources for this population, implementation has lagged. Integrated treatment involves targeting both the substance use and the mental illness at the same time in a systematic way and is a recommended practice for CDs. However, very little true integration of addiction and mental health services has occurred in Canada. This review outlines the main best-practice guidelines for working with people with CDs and describes some of the barriers and facilitators to integration. An example of successful integrated treatment is presented, with suggestions for how psychologists can play a key role in this important work. The prevalence of disordered gambling worldwide has been estimated at 2.3%. Only a small minority of disordered gamblers seek specialist face-to-face treatment, and so a need for alternative treatment delivery models that capitalise on advances in communication technology, and use self-directed activity that can complement existing services has been identified. As such, the primary aim of this study is to evaluate an online self-directed cognitive–behavioural programme for disordered gambling (GAMBLINGLESS: FOR LIFE). Open Access Article
Comment: Opioid substitution therapy (OST) using oral methadone and buprenorphine has consistently been shown to substantially reduce illicit heroin use, opioid overdose deaths, and HIV infection, and to improve the quality of life of people who are opioid dependent.1 OST is usually recommended as a long-term maintenance treatment because its benefits are most apparent while patients remain in treatment, and they rapidly dissipate when patients withdraw from treatment.2 What happens to opioid and other drug use when patients remain on OST for a decade or more? Although the beneficial effects of opioid substitution for the reduction of heroin use are well established, its effect on other substance use is unclear. We aimed to evaluate short-term and long-term changes in substance use in opioid-dependent patients on opioid substitution therapy. We focused on frequent use of heroin, cocaine, benzodiazepines, and alcohol under naturalistic conditions (ie, with non-selected patients and clinical practice as usual) over 17 years. We provide three examples of how addiction stigma is sanctioned: (a) discrimination against people with addictions is often legal; (b) public health communications frequently use stigma to promote prevention; (c) some programmes, such as ‘12 steps’ promote self-stigma.
