Criminal-justice-involved persons are highly susceptible to opioid relapse and overdose-related deaths. In a recent randomized trial, we demonstrated the effectiveness of extended release naltrexone (XR-NTX; Vivitrol®) in preventing opioid relapse among criminal-justice-involved U.S. adults with a history of opioid use disorder. The cost of XR-NTX may be a significant barrier to adoption. Thus, it is important to account for improved quality of life, and downstream cost-offsets. Our aims were to (1) estimate the incremental cost per quality-adjusted life-year (QALY) gained for XR-NTX versus treatment as usual (TAU), and evaluate it relative to generally-accepted value thresholds; and (2) estimate the incremental cost per additional year of opioid abstinence. Clinical guidelines recommend Self-Management and Recovery Training (SMART Recovery) and 12-step models of mutual aid as important sources of long-term support for addiction recovery. Methodologically rigorous reviews of the efficacy and potential mechanisms of change are available for the predominant 12-step approach. A similarly rigorous exploration of SMART Recovery has yet to be undertaken. We aim to address this gap by providing a systematic overview of the evidence for SMART Recovery in adults with problematic alcohol, substance, and/or behavioral addiction, including (i) a commentary on outcomes assessed, process variables, feasibility, current understanding of mental health outcomes, and (ii) a critical evaluation of the methodology. Understanding why people repeatedly attend Emergency Departments (EDs) for alcohol-related reasons is an important prerequisite to identifying ways of reducing any unnecessary demands on hospital resources. We use Andersen’s Behavioural Model of Health Services Use to explore factors that contributed to repeat ED attendances.
While the study cannot prove cause and effect, and stiffening of the arteries can have a range of causes, it does highlight the fact alcohol-related harms can affect anyone.

To test a neurobehavioral model of adolescent substance use disorder (SUD) resulting from an imbalance between a hyperactive reward motivation system and a hypoactive executive control system. Norfolk Recovery Partnership (NRP) is working with homeless hostels across Norfolk to train staff to administer Naloxone, which is used when people have taken heroin and other opiate overdoses. It is hoped that by providing hostels with a take-home kit, as well as the right training on how to use it, the number of people who die prematurely as a result of an overdose will reduce.
NRP, which is a partnership between Norfolk and Suffolk NHS Foundation Trust (NSFT), The Matthew Project and the Rehabilitation for Addicted Prisoners Trust (RAPt), has so far trained staff at many hostels, including Bishopbridge House and the Ripley Project, both in Norwich. A further session will take place with staff from Genesis Housing next month.

Studies of social drinkers indicate that subjective response (SR) to alcohol and impulsivity are risk factors for the development of alcohol use disorder which may be related. It is unclear, however, whether there are significant relationships between SR and impulsivity among individuals with alcohol dependence. Using data from an intravenous (IV) alcohol challenge study, the present study is the first to explore the relationship between impulsivity and SR during alcohol administration among alcohol-dependent individuals.
