Too late to be included in this review, NICE Guideline Depression in adults, was issued in 2022 with more complete tapering information, including an exponential taper to be adjusted to the tolerance of the individual patient. It advised clinicians “slowly reduce the dose to zero in a step-wise fashion, at each step prescribing a proportion of the previous dose” and “consider using smaller reductions”. While this is still not very specific, it is a significant improvement over the guidance reviewed in this paper.
Cytochrome P450 1A2 (CYP1A2) is involved in the metabolism of antipsychotic drugs such as clozapine and olanzapine. Personalisation of these treatments requires an accurate estimation of CYP1A2 activity. To read the full article, choose Open Athens “Institutional Login” and search for “Midlands Partnership”.
Older adults are particularly affected by medication-related harm (MRH) during transitions of care. The PRIME study (prospective study to develop a model to stratify the risk of MRH in hospitalized patients) developed and internally validated a risk-prediction tool (RPT) that provides a percentage score of MRH in adults over 65 in the eight-weeks following hospital discharge. Drug-induced skin disease or cutaneous adverse drug reactions (CADRs) are terms that encompass the clinical manifestations of the skin, mucosae and adnexa induced by a drug or its metabolites. The skin is the organ most frequently affected by drug reactions, which may affect up to 10% of hospitalized patients and occur in 1% to 3% of multi-medicated patients. Drug Hypersensitivity reactions (DHRs) are type B adverse drug reactions (ADRs) traditionally defined as unpredictable, dose independent and not related to the drug pharmacology. This review attempts at summarizing the available evidence to further illustrate the pathophysiology of DHRs.
