Open access. Cardiometabolic health significantly impacts on the mortality of people with severe mental illness. Clozapine has the greatest efficacy for Treatment Resistant Schizophrenia (TRS) but the greatest negative impact on cardiometabolic health. Balancing the risks and benefits of treatment, dignity, autonomy, liberty, mental and physical health can be challenging, particularly when imposing interventions with potentially life threatening adverse events, such as clozapine. We describe the successful administration of clozapine in the face of myocardial infarction, pulmonary embolism and hyperlipidaemia resulting in the termination of long-term seclusion for a gentleman with TRS in high secure psychiatric services.

Tracy and Angela spoke about the Visiting GP Service, which was rolled out across our secure services. The service operates from The Forge GP practice in Warrington and is accessible to all service users staying on our low-secure inpatient wards at Hollins Park Hospital.
Tracy and Angela have also implemented a comprehensive physical health database to assist the service in reviewing, monitoring and optimising the health and wellbeing of our patients. The database captures the whole range of physical health needs, including national screening programmes and the safe management of service users with long-term health conditions.

Vitamin D has an integral role in maintaining calcium homeostasis in the blood. It is well established that prolonged and severe vitamin D deficiency leads to rickets in children and osteomalacia in adults. Increasing evidence indicates that deficiency has associations with a number of other physical and mental health conditions. Doctors should ensure that their patients have information so that they can make informed decisions about their health, be it their physical or mental health. Recent NICE guidelines on vitamin D recommend that health care professionals have a role in informing patients about vitamin D supplements. This is especially relevant for forensic psychiatrists, as their patients often fall into recognised at risk groups for lowered vitamin D levels. The paper, entitled ‘Using the National Diabetes Audit to improve the care of diabetes in secure hospital in-patient settings in the UK‘, highlights new data showing that high-quality diabetic care can be provided to inpatient forensic service users in high, medium and low secure units.
To examine the ability of two forensic units, one high secure and one medium secure, to meet the NICE (National Institute for Health and Care Excellence) standards of care for diabetes. The Physical Healthcare Team at Rampton Hospital, which is managed by Nottinghamshire Healthcare, has been recognised for developing the service at the High Secure Hospital to enable minor surgery to be carried out in the physical healthcare centre on site.

The team identified a need for the service development in particular for a number of patients who self harm by inserting foreign objects into themselves.
