In keeping with Rapoport's Rules of Argumentation,1 we acknowledge several points of agreement with Jonathan Shedler regarding our Comment.2 We concur that the treatments recommended in the post-traumatic stress disorder (PTSD) practice guidelines are not panaceas: even the best PTSD treatments leave many patients with clinically significant symptoms. Development of better interventions or improvement of existing ones to reach these remaining individuals is needed.

In a Comment published in The Lancet Psychiatry, Scott Lilienfeld and colleagues1 wrote that I encouraged psychotherapists to ignore new practice guidelines for post-traumatic stress disorder (PTSD) because they relied inordinately on randomised controlled trials (RCTs). I did not urge therapists to ignore the guidelines because they relied on RCTs; I urged them to ignore the guidelines because research studies—the same RCTs they laud—show that the recommended therapies do not work for most patients.. For the past 60 years, the mainstay of treatment for people with psychotic illnesses has been antipsychotic drugs.1 Antipsychotics, however, are not usually fully effective, and are associated with side-effects ranging from weight gain and metabolic syndrome to the irreversible movement disorder tardive dyskinesia. In the past 20 years, modern psychological interventions have been explored as alternative treatments. Keeping individuals on antidepressants after remission or recovery of major depressive disorder is a common strategy to prevent relapse or recurrence. Preventive cognitive therapy (PCT) has been proposed as an alternative to maintenance antidepressant treatment, but whether its addition would allow tapering of antidepressants or enhance the efficacy of maintenance antidepressant treatment is unclear. In 1994, a randomised controlled trial introduced the sequential design in depression.1 In that trial, individuals with depression who had responded to drug treatment were randomly assigned to cognitive behavioural therapy or to clinical management while antidepressant drugs were tapered and discontinued.1 This design was subsequently used in a number of randomised controlled trials and was found to lead to significant benefits.2 The strategy is an intensive, two-stage approach in which one type of treatment (psychotherapy) is used to improve symptoms that another type of treatment (pharmacotherapy) was unable to affect.. The publication of our updated network meta-analysis1 about antidepressants for the acute treatment of major depression has generated a wide discussion in newspapers,2, 3 social media,4 and scientific journals.5 Based on 522 trials and more than 116 000 patients, this network meta-analysis of 21 drugs and placebo represents the most comprehensive analysis of the evidence base ever undertaken. We take the opportunity of this Comment not to repeat our findings but to reflect on the implications of this research and, most importantly, on what needs to be done next to improve the outcome of patients with major depression.. Comment. In The Lancet Psychiatry, Jesse Fann and colleagues1 report the long-term risk of dementia following traumatic brain injury (TBI) in a national population-based observational study. Using Denmark's enviable and societally accepted comprehensive register systems, the authors could link recorded inpatient and outpatient episodes of TBI with other registry records years and decades later for any type of dementia. This means that the analysis included 27 632 020 person-years of observation of 2·8 million people in 132 000 episodes of recorded TBI during a 36-year period between 1977 and 2013, with dementia outcomes being detected through the same systems for 127 000 of those people between 1999 and 2013..
