Today, in partnership with NHS Confederation, National Voices and the Centre for Public Scrutiny, we have launched a new resource to support those delivering change within their Sustainability and Transformation Plan (STP) footprints to work through key governance and accountability challenges.
CIPFA has analysed nine published STPs out of the 44 that are being developed in England to improve services while reducing costs. It found that although the plans set out proposals to close the widening financial gap, which the NHS estimates to be £30bn by 2020, some fail to set out a credible case for the deliverability of their savings plans.
It is unfortunate that due to the decision by NHS England asking all local teams not to communicate any information about their Sustainability and Transformation Plans (STP), we now see inaccurate reports in the national media.
The STPs have been discussed for several months with health, local government, regulatory bodies and the third sector. The idea has been to collaboratively prepare a credible plan for delivering better services whilst also identifying cost savings. The work around STPs involves the creation of 44 geographical “footprints” across the UK, ours being Shropshire and Telford & Wrekin.

Our idea was to have a single capitated budget (a budget calculated per person) for the whole process, with a common quality specification that had to be owned and delivered by all organisations involved. The financial incentives would be aligned behind commissioner determined common outcomes. The Year of Care capitated budget was our proposal. The idea was to allow care to be provided based on a person’s overall needs as opposed to the specific diseases they have.
The Hospital Transfer Pathway (HTP) – more affectionately known as the red bag initiative – has been one of the major successes of our enhanced health in care homes vanguard to date, and I’ve been thrilled to be a part of its development.
The red bag was born from one of the vanguard’s care home manager forums; a bi-monthly opportunity for care home managers to get together in a safe, informal space, share experiences and discuss issues being experienced in the area. It’s the visual identity of the HTP; a pathway of care that enables care home managers to share important information about residents with hospitals upon admission and discharge. It’s a true example of collaborative working, with involvement from care home staff, the London Ambulance Service, local hospitals and community services.
