There are high levels of comorbidity between mental health and somatic diagnoses. Clinicians must avoid misattributing service users’ new symptoms to the original diagnosis, whilst also being aware of diagnostic overshadowing. These could be indicating a comorbid diagnoses that needs identifying, and without this identification the complexities of each case overall cannot be addressed. Clinicians and service providers should also be aware of the complexities comorbidities create for individuals and their carers, and should aim to provide (or signpost to) cross-sector support where possible.
Clinical guidelines recommend providing physical activity interventions (PAIs) to people with schizophrenia or bipolar disorder for weight management. The integration of smoking cessation treatment in NHS Talking Therapies would help people with mental health conditions who smoke receive evidence-based support for both at the same time and location. However, more funding and staffing of these talking therapies would be necessary if the intervention was implemented. There are already long-wait times of twenty-three weeks (162 days) for an initial consultation for NHS Talking Therapies (NHS, n.d.). Making another population eligible for these services would potentially overwhelm these services and increase wait times. With the evidence in mind, it would be useful for clinicians to gather information on smoking and substance use during the initial mental health assessment and inform their practice.
Massage therapy has recognised psychological benefits, yet it is rarely used in forensic mental health settings. .... Seated chair massage was found to have overwhelmingly positive effects in the short term and the authors concluded that it is a low-risk, boundaried and appropriate complementary therapy intervention for patients in forensic mental health settings.

Although efforts from within mental health contexts are fundamental, it is also essential that a collaborative interdisciplinary approach is taken to address the “multifactorial nature of risk” for premature mortality due to suicide and physical health comorbidities.

Cannabis is used by one third of youth in Early Psychosis Intervention (EPI) programs and high dose consumption of the primary constituent Δ-9 tetrahydrocannabinol (THC) is associated with higher risk for relapse in this group. Cannabidiol (CBD) is a secondary cannabis constituent that may have antipsychotic properties, though its health risks are only beginning to be understood. Little is known about the views of youth in EPI programs toward CBD, including their reasons for use and perceptions of risk.

Certain conditions are seemingly under-diagnosed at the point of schizophrenia diagnosis, possibly resulting from under-investigation or diagnostic overshadowing, which is more common among this group (Jones, Howard & Thornicroft, 2008). This is a widespread issue with data suggesting that at the end of 2021, a mere 35% of people with severe mental illness received annual health checks (NHS England, 2020). This points to the need for improvement in the monitoring, screening and diagnosis of physical health conditions for individuals with severe mental illness.
