A caseload management tool was developed with the aim of creating a straightforward, unbiased way of assessing a clinician's caseload for the community mental health teams (CMHTs) in Aneurin Bevan University Health Board. For the first time, the tool developed allows for individual clinicians to record caseloads electronically, enabling decision makers and their teams to assess caseloads. The newly developed tool also has the capability to assess a client's individual weighting score over time. Many healthcare services are under considerable pressure to reduce costs while improving quality. This is particularly true in the United Kingdom’s National Health Service where postnatal care is sometimes viewed as having a low priority. There is much debate about the service’s redesign and the reallocation of resources, both along care pathways and between groups of mothers and babies with different needs. The aim of this study was to develop a decision support tool that would encourage a systemic approach to service redesign and that could assess the various quality and financial implications of service change options making the consequent trade-offs explicit. The paper describes the development process and an initial implementation as a preliminary exploration of the possible merits of this approach.

The team – based centrally in one location at Leechmere, Grangetown – aims to support adults who live in Sunderland, who are registered with a Sunderland GP and need short term health and or social care support, that can help to keep them living at home, with care wrapped around them while they’re at their most vulnerable.
Support is tailored to a person’s needs and can be any combination of a short term care package, from nursing to therapy to get them back on their feet without having to be hospitalised or needing long term care. GP support is also available within the service.
Through Recovery at Home, those who need greater support while they’re getting back to normal after a short term condition can also be provided with bed-based care, meaning more intensive support can be offered in their own home, including residential or nursing care homes. And those who need to be in a fully supported environment will be given all of the support and advice they need to get them ready to return home and live independently.
This centralised team gives carers, GPs, health and social care staff, as well as other agencies, like the police, an invaluable single point of contact for some of the most vulnerable people in our communities who are at greatest risk of hospitalisation.

Last week we launched ambitious new standards that set out the skills and knowledge the next generation of nurses need. We also introduced a more modern and innovative approach to the way universities and their practise partners train nurses and midwives. The changes will allow greater independence of assessment, and greater innovation by placement providers.

The purpose of this article is to fill the need for an updated concept analysis of compassion fatigue. This review used Rodgers' Evolutionary Model to direct the analysis of 25 papers published between 2005 and 2015. This review focused strictly on healthcare providers in the formal healthcare setting. The authors provided an analysis of the evolution of compassion fatigue and identified surrogate terms, attributes and defining characteristics, and antecedents and consequences of the term, true to Rodgers' Evolutionary Model. Related terms are also discussed and a brief case model is provided. A better understanding of compassion fatigue will allow researchers and clinicians to identify its causes, prevention, and treatment. This could impact the rate at which healthcare providers leave clinical practice and their ability to compassionately engage with patients. The success of this depends on professionals changing practice behaviours. This study explored the feasibility of applying a behavioural science approach to help staff teams from health organisations overcome psychological barriers to change and implement new models of care.
Patient-centred care, defined as respecting and responding to the needs and preferences of patients, empowering them to make decisions that best fit their individual needs, has been identified by the Institute of Medicine as an essential element of high-quality care.1 It can be thought of as respectfully involving the patient2 in a way that helps practitioners provide care that is concordant with their patients’ values, needs and preferences while better enabling patients to actively provide input and participate in their healthcare.3 Patients are more satisfied with their care when they feel that healthcare providers are understanding their needs, carefully listening and clearly providing information4; in addition, patient-centred care has been found to be associated with improved patient outcomes.5 In order to provide exemplary patient-centred care, one needs well developed communication skills, especially in the realm of active listening and responding to patient cues. The importance of physicians mastering the art of patient-centred communication skills can be seen as a theme in the educational objectives of medical school curricula as well as in the competencies of the Accreditation Council for Graduate Medical Education.
