Acute wards for adults of working age and psychiatric intensive care units (PICUs) at West London Mental Health NHS Trust have made good progress in some key areas. The trust had undertaken considerable work to better manage patient flow. This had resolved the issue of patients receiving care on one ward while sleeping on another ward.

No standardised tools for assessing the quality of specialist mental health supported accommodation services exist. To address this, we adapted the Quality Indicator for Rehabilitative care-QuIRC-that was originally developed to assess the quality of longer term inpatient and community based mental health facilities. The QuIRC, which is completed by the service manager and gives ratings of seven domains of care, has good psychometric properties.
Adherence to medicines has been identified as a key issue by the Royal Pharmaceutical Society of Great Britain,1 the WHO2 and, most recently, by the UK Academy of Medical Sciences.3 Estimates of adherence vary widely but are often reported to be as low as 50%.4 When a patient does not take their prescribed medicines as intended, they may not derive the expected outcomes. North Staffordshire Combined Healthcare NHS Trust has today welcomed the publication of its latest CQC inspection, which - for the first time in the Trust's history - rates every Combined Service as "Good" or "Outstanding". The Trust's overall rating is "Good".
The results mean that Combined Healthcare is the best rated mental health trust across the whole of the Midlands and East of England and third highest in the whole country - only 1 of 3 with every service rated at least Good and at least two Outstanding.

Always Events are aspects of the patient experience that are so important to patients and family members that health care providers must aim to perform them consistently for every individual, every time.
There was a strong recovery ethos throughout service delivery. Staff shared a clear definition of recovery and supported clients to achieve their goals. Staff were hard working, caring and committed to delivering a good quality service.
Staff supported clients to engage in their local community. Dedicated workers helped clients’ engagement with community services and worked to bridge the gap in support after treatment and promote independence and self-care in clients.
Staff used effective systems to identify and manage client risk. Safety was a priority in all teams. The whole team was engaged in reviewing and improving safety and safeguarding systems. There were effective systems in place to ensure that safeguarding concerns were identified, managed and reviewed.

AQUA - Automatic Quality Assessment and Feedback in eLearning 2.0
The current development of Web 2.0 makes the distinction between author and reader fading away. Users now produce huge amounts of data which sometimes is of questionable quality. This leads to the problem of information overload: how to make the most of this information without overwhelming the users? One key challenge to solve this issue is to assess the quality of the user generated content.
In AQUA, we seek to develop algorithms to assess the quality of content automatically. We focus on two sources for this assessment: (1) user generated content; (2) feedback by users of the content. To do so, we investigate techniques from the fields of natural language processing (NLP), information retrieval, and machine learning.
So, in a nutshell, AQUA will answer the following questions:
What is quality of information? How does it matter in information search?
How to model the quality of user generated content?
How far can you go with automatic methods in assessing quality?
How to give feedback to users regarding quality?
The AQUA project is associated with the project "Mining Lexical-Semantic Knowledge from Dynamic and Linguistic Sources and Integration into Question Answering for Discourse-Based Knowledge Acquisition in e-learning (QA-EL)".

Prior literature identified the use of Performance Measurement Systems (PMS) as crucial in addressing improved processes of care. Moreover, a strategic use of PMS has been found to enhance quality, compared to non-strategic use, although a clear understanding of this linkage is still to be achieved. This paper deals with the test of direct and indirect models related to the link between the strategic use of PMS and the level of improved processes in health care organizations. Indirect models were mediated by the degree of perceived managerial discretion.

Patient-centred care, defined as respecting and responding to the needs and preferences of patients, empowering them to make decisions that best fit their individual needs, has been identified by the Institute of Medicine as an essential element of high-quality care.1 It can be thought of as respectfully involving the patient2 in a way that helps practitioners provide care that is concordant with their patients’ values, needs and preferences while better enabling patients to actively provide input and participate in their healthcare.3 Patients are more satisfied with their care when they feel that healthcare providers are understanding their needs, carefully listening and clearly providing information4; in addition, patient-centred care has been found to be associated with improved patient outcomes.5 In order to provide exemplary patient-centred care, one needs well developed communication skills, especially in the realm of active listening and responding to patient cues. Independent research carried out by AvMA shows that the Care Quality Commission (CQC) “requires improvement” in how it regulates the statutory duty of candour. This is despite significant improvements since AvMA’s previous report in 2016.

These are the potential stakeholders for QI and its evaluation, and they have valuable perspectives to offer when they are consulted in planning, conducting and interpreting evaluations. QI practitioners are accustomed to consulting stakeholders to assess unintended consequences or assess patient experiences of care, but in many cases there are additional benefits to a broad inclusion of stakeholders. These benefits are better adherence to ethical standards, to assure that all legitimate interests take part, more useful and relevant evaluation information and better political buy-in to improve impact. Balancing various stakeholder needs for information requires skill for both politics and research management. These challenges have few pat answers, but several preferred practices, which are illustrated with practical examples

Barts Health’s ‘Safe and Compassionate’ plan recognises the enormous value of staff contributing to improvements in their services. This is underpinned by Listening into Action, and supported by training for staff in Quality Improvement.
We started by providing a three day course ‘Introduction to Quality Improvement’, initially assisted by the AQuA academy. This includes both a toolkit based around the Model for Improvement including good measurement techniques, sustainability and spread, and some supporting theoretical concepts including the role of human factors in error, stakeholder and team engagement and resilience.
Those attending include members of teams who are actively involved in delivering an improvement and prospective improvement champions.

An enduring challenge for the improvement of healthcare quality is variation in the success of quality improvement (QI) interventions when implemented across settings.1 This is particularly true in the field of healthcare-associated infection (HAI) prevention. Some of the brightest success stories in QI have emerged from large-scale efforts to reduce HAIs such as central venous catheter-related bloodstream infections (CRBSIs)2 or catheter-associated urinary tract infections.3 The light dims, however, when efforts to export these interventions to other settings fail to meaningfully improve outcomes.4 5
Open access. Despite taking advantage of established learning from other industries, quality improvement initiatives in healthcare may struggle to outperform secular trends. The reasons for this are rarely explored in detail, and are often attributed merely to difficulties in engaging clinicians in quality improvement work. In a narrative review of the literature, we argue that this focus on clinicians, at the relative expense of managerial staff, has proven counterproductive. Clinical engagement is not a universal challenge; moreover, there is evidence that managers—particularly middle managers—also have a role to play in quality improvement.
