The majority of people with asthma in the UK are usually managed in primary care following the British Thoracic Society/Scottish Intercollegiate Guidelines Network (BTS/SIGN) asthma guidelines.1 However, not all patients' asthma is well controlled on standard monotherapies of inhaled corticosteroids (ICS) plus short-acting or long-acting bronchodilators.2 Here we examine the role of tiotropium as add-on therapy when treatment needs to be escalated. Neuropsychiatric signs and symptoms occur frequently in individuals with multiple sclerosis (MS), either as the initial presenting complaint prior to a definitive neurological diagnosis or more commonly with disease progression. However, the pathogenesis of these comorbid conditions remains unclear and it remains difficult to accurately elucidate if neuropsychiatric symptoms or conditions are indicators of MS illness severity. Furthermore, both the disease process and the treatments of MS can adversely impact an individual's mental health. In this review, we discuss the common neuropsychiatric syndromes that occur in MS and describe the clinical symptoms, aetiology, neuroimaging findings and management strategies for these conditions. Based on small trials, cardioselective beta-blockers did not seem to cause adverse respiratory effects in patients with COPD of a range of severities, some of whom also had indications for beta-blockers (angina, hypertension) and others who were given beta-blockers experimentally to test effects on lung function.

Effective Primary Care has four central features;
1. The first point of contact for all new health needs.
2. Person-centered (holistic), rather than disease-focused, continuous lifetime care.
3. Comprehensive care provided for all needs that are common in a population.
4. Co-ordination and integration of care when a person’s need is sufficiently uncommon so to require special services or provision from another sector (secondary or tertiary care).

Paediatric Chronic Fatigue Syndrome (CFS)/Myalgic Encephalomyelitis (ME) is relatively common and disabling. Improving treatment requires the development of Patient Reported Outcome Measures (PROMs) that enable clinicians and researchers to collect patient-centred evidence on outcomes. Health professionals are well placed to provide clinical insight into the condition, its treatment and possible outcomes. This study aimed to understand the perspectives of UK specialist paediatric CFS/ME health professionals and identify outcomes that are clinically important.
