Does anyone remember a time when shifting the balance of care into community settings and away from hospitals was NOT a strategic goal for the NHS? A new report from the Fund on the state of district nursing – due to be published later this month – will throw some light on the success (or lack of success) in delivering this goal.
to establish the accuracy of community nurses’ predictions of mortality among older people with multiple long-term conditions, to compare these with a mortality rating index and to assess the incremental value of nurses’ predictions to the prognostic tool.. "The authors concluded: –
Fluconazole and amphotericin B are recommended as topical antifungal agents for adults with oral candidiasis. Existing studies tend to recommend fluconazole for immunocompetent patients and itraconazole for immunosuppressed patients, whereas miconazole is recommended for infants."
Editorial. Alongside concern about avoidable mortality, one of the key findings of the public enquiry into failings at Mid Staffordshire NHS Foundation Trust,1 which ran Stafford Hospital in England, was the lack of compassion in care delivery. Sir Robert Francis, who led the enquiry, laid the blame for the compassion deficit at the door nursing and support staff. He recommended, among other things, that people should work as care assistants prior to nurse training and that values-based recruitment should be used to ensure that the ‘right’ people are recruited to be nurses. However, there has been little evidence to support these propositions. For example Snowden et al2 found that nursing students who had previous care jobs scored no higher for emotional intelligence than those without prior experience. Compassion has historically been defined as an underpinning principle of work conducted by health professionals, especially nurses.1 Numerous definitions of compassionate care exist, incorporating a range of elements. Most include a cognitive element: understanding what is important to the other by exploring their perspective; a volitional element: choosing to act to try and alleviate the other’s disquiet; an affective element: actively imagining what the other is going through; an altruistic element: reacting to the other’s needs selflessly; and a moral element: to not show compassion may compound any pain or distress already being experienced by the other.....To read the full article, log in using your NHS OpenAthens details.

As a result of auditing stoma prescribing practices and the effect on both patients and health economy, GP’s invested money in the colorectal service to fund a clinical nurse specialist to review stoma patients in 17 GP surgeries (Williams, I Gastrointestinal Nursing Sept 2013 Vol 11, Issue 7, 36-44)
The reviews focussed on determining if GP’s were repeat prescribing for individual patients in an appropriate manner, the patients received the most appropriate products and the correct monthly amounts were administered. Although the work aimed to identify the cost benefits associated with a review and rationalisation of patient prescriptions, ensuring timely and expert follow up was a key quality driver.
In total 429 patients were reviewed in 17 surgeries and there were significant cost savings demonstrated in some of the reviews.
The annual stoma costs for the surgeries prior to reviews was £531,694.86 and there was total potential annual savings of £115,125.10.

New research shows cuts to spending in health and social care are linked to 120,000 excess deaths in England.
The critical factor in these figures may be changes in nurse numbers, say the researchers, who warn that there could be an additional toll of up to 100 deaths every day from now on.

In 2016, the Wolverhampton Transfer of Care Project reported that avoidable discharge delays were being caused by a lack of understanding regarding intermediate care settings and difficulty in accessing these services.
They estimated that better use of an integrated service could release 318 bed days each month. They recommended an integrated model of intermediate care which would enable patients to be discharged at the point they were medically ready, and assessed either at home or in an intermediate care setting – the Discharge to Assess (D2A) model.
Our report, Understanding quality in district nursing services, sets out a framework that describes what ‘good care’ looks like in district nursing services. We invited Rebecca, a community staff nurse in London, to share her thoughts on the subject.
Listening to the voices of nurses working closest to patients and supporting them to implement positive change is a way to flatten traditional NHS hierarchies. Done properly, it can improve patient outcomes as well as increase job satisfaction for staff say nursing leaders.
The aim of this study was to find out if student nurses feel comfortable in caring by providing support for Lesbian, Gay, Bisexual or Questioning adolescents and what factors influence their level of comfort. For over twenty years there have been papers highlighting the use of complementary and alternative medicine (CAM). The use in children is increasing across the world and estimated at between 10% and 89%. There is a wide belief they are safe. We know that doctors do not routinely ask parents about use in clinics and we also know that parents don’t want to inform doctors as they don’t think that the doctor will know about the particular CAM or indeed be interested. Farrington et al. [Acta Paediatr 2019;108: 419–422. doi:10.1111/apa.14595] present a simple collection …. To read the full article, log in using your NHS OpenAthens details.
Editorial. For a disease that is eminently preventable, the focus of childhood dental decay has tended to be treatment orientated and siloed as the responsibility of the dental profession. While contemporary evidence continues to support the effectiveness of prevention and early intervention, dental attendance before the age of 2 years is uncommon. However, contact with other health professionals during a child’s first 2 years of life is high. With this in mind, this editorial introduces a series of articles that outlines why childhood dental decay is everyone’s business, the nature and extent of childhood oral health issues and how we could be addressing the complex aetiology of early childhood caries through a progressive, collaborative approach.. The 2014 English Special Educational Needs and Disability (SEND) Code of Practice, a policy framework for delivery of a range of services including Schools Mental Health Services (SMHS), is considered in the context of international research on the delivery of mental health services in schools. Effective interventions have been reported in poor and middle-income countries, whereas there is less information about routine services in high-income societies, and what they report are equivocal outcomes. In England and the United States, problems with acceptability of routine mental health assessment, together with conflict over resource allocation significantly impede both effectiveness and acceptability. The new Code has been specifically written to support better engagement between Education and Mental Health, but piloting suggests that this has not yet happened. Proactive multiagency planning within the framework of the Code, linked to explicit work on organizational transformation, could provide an effective and acceptable Schools Mental Health Service, with legal protection from subsequent deleterious policy and funding changes. Explicit recommendations for using the Code are appended. Login using your SSSFT NHS OpenAthens for full text. SSOTP - request a copy of the article from the library - www.sssft.nhs.uk/library

Compassion has become a topic of increasing interest within healthcare over recent years. Yet despite its raised profile, little research has investigated how compassionate care is enacted and what it means to healthcare professionals (HCPs). In a grounded theory study, we aimed to explore this topic from the perspective of people working with patients with type 2 diabetes – a long-term condition that involves repeated interactions with HCPs.
