Substance use disorder is one of the most stigmatized health conditions. Stigma internalization is one of the main consequences of the stigmatization process, and it is associated with lower self-esteem and self-efficacy and worse recovery prospects. It may also bring guilt, hopelessness, anxiety, self-devaluation, and depression. By developing a comprehensive intervention for primary care staff working at community health centres (CHCs) aimed at reducing stigma towards people with mental health and substance use problems (MHSUP), we sought to test an innovative and contact-based intervention consisting of staff training, raising awareness, a recovery-focused art programme and an analysis of internal policies and procedures.

Two conditions that are highly stigmatized and that can be found in combination with one another are the stigma of substance use and HIV-related stigma. In this qualitative study, we investigated 15 substance users’ experiences surrounding their HIV status from their diagnosis to their disclosure decisions and the reactions from others. We explored the layering of substance use stigma with HIV-related stigma, the consequences of stigmatizing experiences, and coping strategies employed to deal with stigmatization. Three-quarters of people with an alcohol use disorder in the USA never receive treatment. Our understandings of who receives care are informed by sociological perspectives, theories and models, each of which discuss the role of lay people's understanding of illness. However, comparatively little work has been done to unpack the cognitive processes underlying lay assessment. In the context of the Framework Integrating Normative Influences on Stigma (FINIS), we aim to understand key factors guiding lay people's stigmatizing attitudes, perceptions and assessments of alcohol use disorder behaviors.
