After more than 30 years of research, numerous studies have shown that injection drug use is associated with a wide range of adverse health outcomes such as drug overdoses, drug-related suicidal behaviours, comorbid psychiatric disorders, blood-borne pathogens and other infectious diseases, and traumas. This review explores new trends and prominent issues associated with injection drug use. The dynamic nature of injection drug use is underlined by examining its recent trends and changing patterns in Canada and other ''high-income countries.'' Three research topics that could further contribute to the development of comprehensive prevention and intervention strategies aimed at people who inject drugs are also discussed: risk behaviours associated with the injection of prescription opioids, binge injection drug use, and mental health problems as determinants of injection risk behaviours. Adolescent participation in risky and addictive behaviours, such as smoking, substance use, and gambling has the potential to lead to many serious problems. The presence of conduct problems (CPs) and early initiation into risky and addictive behaviours have been independently shown to be associated with adolescent and young adult smoking, drinking, and gambling. Nevertheless, the relation between early initiation into risky and addictive behaviours and CPs remains to be explored among pre-adolescents. Our study aims to examine the prospective relation between CPs in early primary school and pre-adolescent initiation into smoking, alcohol use, and gambling. Previous cross-sectional research has shown that depression and problem gambling co-occur. Longitudinal research, however, allows for a better determination of directionality, as behavioural changes in gambling involvement can be more reliably studied over time. Our study assesses symptoms of depression and problem gambling across 4 waves and addresses whether their relation is directional (with one reliably preceding the other), bidirectional, or pathoplastic. Individuals with attention-deficit/hyperactivity disorder (ADHD) are at increased risk for experiencing alcohol-related problems by adulthood. However, few studies have examined contextual factors that may contribute to this risk. There are fewer data on dental disease in these conditions, in spite of high rates of dry mouth (xerostomia), as well as the associated indirect or life-style effects such as poverty and lack of access to care. In an emergency department (ED) setting, a 1-hour motivational interviewing session involving personalised risk feedback can yield meaningful reductions in hazardous alcohol use and risky sexual behaviour.
Research is needed to evaluate this intervention with racial, ethnic and sexual minority patients and test its effectiveness when delivered by clinical rather than research staff.
Additional innovation is needed to develop integrated brief intervention protocols for addressing a larger range of health risks and to increase the feasibility of implementing such protocols in all appropriate settings. To read the full article, log in using your NHS OpenAthens details
Smoking is an important risk factor for mental health-related problems. Numerous studies have supported a bi-directional association between cigarette smoking and depression. Despite the advances in understanding the comorbidity between both problems, the most effective psychological treatment that simultaneously targets smoking and depressive symptomatology remains unclear. The objective of this study is to assess the effectiveness of a cognitive-behavioral intervention for smoking cessation with components of behavioral activation for managing depressed mood.

Comment. Given the growing prevalence of cannabis use and trend towards legalisation, questions regarding potential biological and mental health effects are increasingly pertinent. The retrospective cohort study by Agrawal and colleagues1 in the Lancet Psychiatry begins to address these questions, in particular with regard to effects of cannabis smoking on depression and suicidal behaviours. Understanding the relationship between smoking cannabis and mental health requires careful examination of genetic and environmental factors. The concept of “musical addictivity” has previously been proposed, arguing that music activates the reward centers of the brain and thus can lead to behavioral addiction. Recent studies support this idea for music consumption. However, there has not been any research on whether these findings could be transferred to music practicing so far. Anecdotal evidence has shown that some musicians either continue to practice through practice-induced pain or have psychosomatic disorders at deprivation, thus transforming a former goal-directed behavior into a maladaptive one. In the present study, the hypothesis of music practice being addictive was first evaluated on a theoretical level taking into account psychological, clinical, and neuroscientific evidence. Open access. Aims and method We examined non-medical use (NMU) of olanzapine among adults on methadone treatment. Information was collected on patient demographics and NMU of olanzapine. The Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) was administered to assess risk among current users of olanzapine.
Results Ninety-two clients participated and 30% reported lifetime history of NMU of olanzapine. Nine people reported doses of 30 mg or higher on a typical day of use, with three typically using 100 mg. The most common reasons for use were to relieve anxiety and to aid sleep, but a quarter used it to ‘get stoned’. Eleven participants (12%) reported NMU of olanzapine in the preceding month. Eight completed the ASSIST with four scoring in the high-risk zone.
Clinical implications Self-medication is the dominant motivator for NMU of olanzapine, but hedonic motivations also occur. A small minority show features of dependency. All doctors should be aware of the potential NMU of olanzapine, especially among patients with history of addiction.
But these findings come with a number of cautions. The researchers say they can't prove smoking caused the gene expression variations, or that these variations cause cannabis, alcohol or tobacco use in later life.

Alcohol Brief Intervention (ABI) uses a motivational counselling approach to support individuals to reduce excessive alcohol consumption. There is growing evidence on ABI’s use within various health care settings, although how they work and which components enhance success is largely unknown. This paper reports on the qualitative part of a mixed methods study. It explores enablers and barriers associated with alcohol reduction following an ABI. It focuses on alcohol’s place within participants’ lives and their personal perspectives on reducing consumption. There are a number of randomised controlled trials in this field though few ABI studies have addressed the experiences of hazardous/harmful drinkers. This study examines factors associated with alcohol reduction in harmful/hazardous drinkers following ABI.

Although clear benefits are associated with reducing smoking, there is increasing pressure on public health providers to justify investment in tobacco control measures. Decision makers need tools to assess the return on investment (ROI)/cost effectiveness of programs.
